US Youth Soccer Concussion Protocol and Notification
You should become familiar with US Youth Soccer's Concussion Protocol and if an incident occurs be using the Concussion Notification Form.

- Concussion Notification Form - This should be used to provide a notice to the parent or guardian that their child might have suffered a concussion during training or a match. Please sure to keep a copy for your files as well as giving one to the parent or guardian.
